Terms & Conditions. Carefully read these terms and conditions of use (“Terms of Use”) as they govern your access to and use of iBiome Healthcare (“iBiome Healthcare” “we,” “us,” and “our”) platform. Your acceptance of, and compliance with, these Terms of Use is a condition to your use of the following: our “Website”. Any supplements (“Supplements”) available for purchase through our website; and services (“Services”) made available through our website.
HIPPA Privacy Practices. You agree that information provided by you in connection with the Website and its Services (Patient Portal) shall be governed by this privacy policy, which is hereby made part of these Terms of Use.
Not For Emergencies. Our website and its services are not for medical emergencies or urgent situations. You should not disregard or delay seeking medical advice based on anything that appears or does not appear on our website. If you believe you are experiencing an emergency, call 9-1-1 immediately.
Primary Care. We require and recommend that you continue to consult with your primary provider and other healthcare professionals for all your primary care needs. We recommend that you always seek the advice of a physician or other qualified healthcare provider concerning questions you have regarding a medical condition and before stopping, starting, or modifying any treatment.

Collaboration: We are happy to collaborate with your other providers, should you request this, you must sign an additional consent in accordance with this request.
iBiome Healthcare: Fees and Services
· All fees are due prior to your patient visit.
· Based on the time our provider spends with and on each individual patient, in order for us to continue our business model, please take note of the updated fees for service on our website www.iBiomeHealthcare.org
· All new prescriptions and prescription refills require a patient appointment.
· We highly recommend that all first visits be a Concierge in-person visit. Please visit our website for the fees in accordance with this. Some exceptions may be made.

Cancellation policy:
· All cancellations less than 48 hours before a scheduled appointment, will NOT be subject to a refund.
· Cancellations at least or greater than 48 hours prior to your appointment will be refunded 50% of the full appointment fee.
· Rescheduled appointments (within the same week) will not be charged.
· Rescheduled appointments will be charged at the 50% rate.
· All cancellations must be made via the Patient Portal, please do not contact our provider directly.
Risk of Telehealth Services:
· In 2015, AB 809 revised the informed consent requirements relating to the delivery of health care via telehealth by permitting consent to be made verbally or in writing,
· Providers using telehealth technologies to provide care to patients located in California must be licensed in California.
· Patients must be a California resident to be a patient at iBiome Healthcare
· Providers are held to the same standard of care, and retain the same responsibilities of providing informed consent, ensuring the privacy of medical information, and any other duties associated with practicing Nursing regardless of whether they are practicing via telehealth or face-to-face, in-person visits.
· By agreeing to Telehealth services, you acknowledge the potential risks associated with telehealth services. These include but are not limited to the following: information transmitted may not be sufficient (e.g. poor resolution of images) to allow for appropriate medical or health care decision making by the Provider; delays in evaluation or treatment could occur due to failures of electronic equipment; a lack of access to your medical records may result in adverse drug interactions or allergic reactions or other judgment errors; although the electronic systems we use incorporate network and software security protocols to protect the privacy and security of health information, those protocols could fail causing a breach of privacy of your health information.
HIPPA Compliant Communication-Patient Portal.
· Email and text are not HIPPA compliant and place your health privacy in jeopardy.
· By agreeing to become an iBiome Healthcare patient, you must agree to maintain all communication via our patient portal, which is HIPPA compliant.
Continuity and commitment to appointments: Equal participant in your healthcare:
· Your satisfaction and good outcomes mean a lot to us. We believe you are saving money by preventing future illness in working with us.
· To continue receiving the greatest benefit of our services, support for diet and supplement recommendations, patients are highly recommended to maintain their appointments at the minimum, on a monthly basis (until directed otherwise by our provider).
· For patients who see us every few months, this is akin to the conventional medical model and does not yield the same outcomes.
o When we see our patients once every few months, we are not servicing you well. We practice lifestyle healthcare, for this to be successful – continuity and consistency is necessary for our model of care.
· Patient Portal Journals: All journals that are filled out in the portal and labwork that is received will be reviewed by our provider, however all follow up actions will be addressed during your appointment time.
Nurse Practitioner Disclaimer:
· Our Nurse Practitioners are not Medical Doctors.
· Our Nurse Practitioners function under Standardized Procedures within their own scope of practice as dictated by the California Board of Registered Nursing, with a Collaborative Physician in the supervisory role.
· I have read and understand that I am being treated by a Nurse Practitioner, and not a Medical Doctor.
Supplements: *According to the Federal Food, Drug, and Cosmetic Act, as amended, Section 201(g)(1), the term drug is defined as an “article intended for use in the diagnosis, cure, mitigation, treatment, or prevention of disease.” Technically, vitamins, minerals, trace elements, amino acids, herbs, or homeopathic remedies are not classified as drugs. However, these substances can have significant effects on physiology and must be used rationally.
Use of nutritional supplements may be safely recommended for patients already using pharmaceutical medications (drugs), but some potentially harmful interactions may occur. For this reason, please consult your health care provider before making any health care decisions or for guidance about a specific medical condition.
You are not required to purchase supplements at our dispensary, we make recommendations & earn a small profit from our holistic dispensary, you are free to purchase them outside our practice and dispensary.
